HOW TO APPLY

Admission form can be obtained from the office of Principal, Public Health School Karachi, Hyderabad, Sukkur, Larkana, and Mirpurkhas. on Payment of Rs. 100/-. Attested Photostat copies of the following documents should be attached with application form:-

a) Matric pass certificate showing the date of birth b) Pass Certificate F.Sc (Premedical) c) Marks Sheet matric / F.Sc (Premedical) d) Domicile Certificate and PRC form-C e) Character Certificate from the School / Institution laste attended OR from any notable. f) CNIC g) Four (4) Passport size photographs duly attested from the back.
